Registration information

Looking to access video content from the conference? Purchase a post-conference recording pass or upgrade your standard pass to a premium pass — both are $50. Please reach out to Alice Smart at meetings.admin@horizonmeetings.com to purchase or upgrade.

Though we were hoping to be able to meet in-person this year, a virtual event means that we can offer registration at a fraction of our usual conference price. Prices start at $99 to attend and participate in the two-day event. For $149, attendees will be able to access recordings of all sessions after the event.

Discounts for students, community health workers, peers, and navigators, as well as free registration for complex care consumers and caregivers are available. See the table below for more information.

If you have questions about pricing or cannot afford to attend but would like to, please contact Alice Smart at meetings.admin@horizonmeetings.com and we'll do our best to accommodate you!

Cancellation policy: All cancellations must be received in writing and sent to meetings.admin@horizonmeetings.com. If cancellation is requested on or before October 2, 2020, you will receive a full refund. After October 2nd, registrations will not be refunded but you will be granted access to all session recordings. All refunds will be processed post-conference. 

Definitions for discounted and free registrations

Community health workers/Peers: includes anyone who currently holds a job title and/or certification as a Community Health Worker (CHW) or Peer Specialist. If your title or certification is a variation of CHW/Peer we will verify that it still falls within the common definition of these roles. This rate is offered to individuals based on their current job title or certification as a CHW/Peer, not the nature of the organization they work for.

  • Must submit photo/document that shows a current professional title and/or certification as a CHW/Peer (or similar title that falls within the common definition of these roles)

Health navigator/Peer health navigator: includes anyone who currently holds a job title and/or certification as a Health/Healthcare/Peer Health Navigator. If your title or certification is a variation of Health Navigator we will verify that it still falls within the common definition of this role. This rate is offered to individuals based on their current job title or certification as a Health Navigator not the nature of the organization they work for.

  • Must submit photo/document that shows a current professional title and/or certification as a Health Navigator (or similar title that falls within the common definition of this roles)

Student: Anyone with a valid student ID or transcript 

  • Must submit photo/document that shows a current student ID or transcript

Consumer/Caregiver:

  • We define consumers and caregivers as people who have lived experience with complex health and social needs,or people who are caregivers for someone with complex health and social needs. "Complex health and social needs" includes things like living with multiple health conditions, people living with disabilities and people with experiences of poverty, homelessness, substance abuse, incarceration, etc. Do you identify as a person with lived experience with complex health and social needs, or the caregiver of a person with complex health and social needs? 

  • We can only offer this reduced rate with the assumption that those who are able to pay the regular registration rate will do so. This includes people who could afford to pay the rate themselves and those who could ask an organization that they work for or are affiliated with to pay for them to attend. Have you already considered whether or not you can afford to pay and whether there is an organization you are affiliated with that could pay for you to attend?

  • After looking into options to pay the regular registration rate, are you still in need of this discounted rate in order to attend?

Must answer yes to all three questions to be eligible for the free consumer/caregiver rate
