Advisory Councils Transformed (ACT): Creating meaningful partnerships with patients, families, and communities

PFCCpartners will host a pre-conference session on creating meaningful partnerships with patients, families, and communities on Tuesday, October 14, from 12:30 - 4:30 pm. Patient and family engagement (PFE) is often championed as a priority in healthcare, yet organizations struggle to translate this vision into transformational, lasting impact. Many health systems, especially those in rural and primary care settings, have limited resources to sustain traditional Patient/Family Advisory Councils (PFACs). PFACs that do exist are often challenged to fully represent the diverse patient populations the organizations serve, and their impact limited by organizational structures that isolate their activities. This pre-conference session will introduce the Advisory Councils Transformed (ACT) Framework - a foundational approach to inclusive engagement, expanding impact, and person-centered transformation. 

The ACT Framework better meets the needs of diverse lived-experience partners through inclusive lanes of engagement. These engagement pathways can integrate with existing PFAC infrastructure or be used as direct pathways for engagement without convening of a formal PFAC. The ACT Framework allows healthcare teams to use their engagement resources more efficiently and to expand the impact of their lived-experience partners by embedding their contributions more directly into improvement efforts. Most importantly, the ACT Framework establishes more inclusive pathways for collaborating with historically underrepresented voices.

Through facilitated brainstorming, strategic planning, hands-on activities, and interactive table discussion, participants will develop actionable strategies for engaging people with lived experience directly into healthcare improvement work. Join PFCCpartners to catalyze progress in patient/family engagement, one of the most influential drivers of health equity, policy change, and patient outcomes.

Complex care essentials: Principles, partnerships, and practice

This 3-hour pre-conference session, hosted by the Camden Coalition, offers a high-level overview of the foundational principles and structural components of complex care. Taking place on Tuesday, October 14, from 2:00 - 5:00 pm, this session is designed for care team members, supervisors, program managers, administrators, and system leaders who are new to the field or seeking to build or broaden their understanding. Topics will include person centered approaches, health-related social needs (HRSN), harm reduction, trauma-informed care, and the importance of authentic healing relationships. The session will highlight practical tools for reflective supervision and strategies to build trust with communities, elevate lived experience, and develop sustainable community engagement and leadership structures like advisory committees.

In addition to front-line practices, the session will touch on systems-level components like backbone organizations, governance models, and payment mechanisms such as 1115 waivers and value-based care. The role of data, continuous learning, and quality improvement will also be discussed. 

Real-world examples and introductory tools will be shared throughout to support understanding and future exploration. This session is intended to lay the groundwork for deeper conversations and learning throughout the rest of the conference.

Site visit opportunities

Back by popular demand! On Thursday afternoon, all Putting Care at the Center 2025 attendees will have the opportunity to participate in site visits to community programs, local organizations, and cultural highlights as part of the conference agenda. These site visits will showcase Portland's vibrant ecosystem of health and social care and offer attendees the chance to forge new connections and enrich their conference experience beyond the hotel. 

Site visits will take place on Thursday, October 16, between 2:45 - 5:30 pm and are included in your registration—there is no additional cost to participate. Pre-registration is required, and space is limited.

Asian Health & Service Center (AHSC)

Asian Health & Service Center (AHSC) has worked to meet the comprehensive needs of Asian communities in the Portland Metro area for over 40 years, with a mission to be the bridge between Asian and American cultures and build a harmonious community. AHSC’s programs and services are tailored to meet the cultural and language needs of the four largest Asian ethnic groups in Oregon: Mandarin-speaking Chinese, Cantonese-speaking Chinese, Vietnamese, Korean, and English speakers.

This site visit will include a tour of AHSC’s House of Love and Kindness in SE Portland and will be a chance to learn about culturally and linguistically specific services available in four key areas: community engagement, behavioral health care, public health and social services, and cultural and educational programming.

Central City Concern (CCC)

Central City Concern (CCC) is Portland's largest provider of healthcare, housing, employment, and recovery services for people experiencing homelessness. On this site visit, attendees will see recent CCC program innovations to "move the front door of healthcare" to make services more accessible and relevant to CCC clients. Attendees will gain a nuanced understanding of the acuity and severity of Portland's homelessness crisis and take away ideas and examples for effective nonprofit programs replicable in every community.

Lift Urban Portland (Lift UP)

Lift Urban Portland (Lift UP) is passionate about making nutritious food easily accessible to their low-income neighbors in downtown and northwest Portland. The organization approaches food security holistically, adapting to meet dietary restrictions and cultural needs, and providing the tools, education, and resources their community needs to prepare food from Lift UP. 

This site visit will include a tour and an opportunity to learn about Lift UP's Adopt a building Program. This program serves residents across 50 affordable housing buildings in the heart of Portland's city center. Services include delivered groceries, on-site pantries, and a cooking and nutrition education class called Supper Club.

New Avenues for Youth

New Avenues for Youth is a Portland-based nonprofit organization dedicated to the prevention and intervention of youth homelessness. Their services for young people (ages 9-25) focus on the individual-their experiences, identities, needs, and goals-and helping them make positive changes in their lives. 

This site visit will include a tour of New Avenues for Youth's Drop-In Day Services, Education and Employment, and Drug/Alcohol Recovery and Mental Health programs. New Avenues staff will offer information on their programs, how they are engaging with youth in the Portland community, and will facilitate open discussion for Q&A.

Oregon Change Clinic

Oregon Change Clinic (OCC) is a culturally specific, outpatient treatment facility and supportive housing provider for mental health treatment and substance use recovery. OCC specializes in working with Black, Indigenous, People of Color (BIPOC) communities within Portland, Oregon, upholds a trauma-informed approach to care, and is a provider of Transcranial Magnetic Stimulation (TMS), a non-invasive procedure to treat symptoms of depression. 

This site visit will include a tour of OCC's outpatient treatment facility and supportive housing building, offering an opportunity for conference attendees to learn more about OCC's work in a clinical and lived experience context.

Oregon Museum of Science and Industry (OMSI)

Explore science-fueled fun at Portland’s premier science museum! With interactive exhibits, hands-on learning activities, science labs, and more—Oregon Museum of Science and Industry (OMSI) is the perfect spot to spark your curiosity.  October through February, OMSI will feature Monsters of the Abyss, a traveling exhibition that allows visitors to dive into the history of amazing prehistoric and modern day aquatic predators and learn about their ecology and why they were so feared.  The exhibit will feature live animal habitats, life-size skeleton replicas and real fossils.

Portland Art Museum

The Portland Art Museum (PAM) is the oldest art museum in the Pacific Northwest, located in downtown Portland. It houses a diverse collection spanning various art forms and historical periods, including notable holdings of Pacific Northwest art, Native American art, Asian art, and post-WWII American photography. The museum also features a center for modern and contemporary art, and its campus is currently undergoing a significant transformation project to enhance accessibility and create new gallery spaces.

Rose Haven

Rose Haven is a day shelter and community center serving women, children and gender non-conforming folks experiencing the trauma of abuse, loss of home, and other disruptive life challenges. They break the cycle of homelessness by providing meals, clothing, first aid, mailing addresses, hygiene, restrooms, showers as well as educational programs and guidance through medical and social services. By meeting basic needs and building trust, they empower their guests to explore long-term change.

The site visit will include a walk through of their trauma informed space, hearing about their services and service model, and include an opportunity for Q&A.

Travel notes: Attendees will have the option of walking (approximately 22-minutes from the Hilton Portland Downtown) to Rose Haven or taking the MAX Blue Line, led by staff, from the Pioneer Square North stop (approximately 3-minute walk from Hilton Portland Downtown) to Providence Park stop and walking 9-minutes to Rose Haven. Attendees will be on the move for much of the tour. The meeting location and other logistics will be provided to attendees who sign up for this site visit. The Native American Rehabilitation Association of the Northwest (NARA NW)

The Native American Rehabilitation Association of the Northwest (NARA NW) is a non-profit agency providing culturally appropriate services to Native Americans, Alaska Natives, and anyone in need through education, health, mental health, substance use recovery, and social services.

This site visit will include a tour of NARA Administration, Child & Family Services building located approximately 30-minutes outside of Portland in Gresham, OR. Attendees will see NARA NW’s youth residential facility, on-site garden, and have the opportunity for Q&A with staff of NARA NW’s various programs within the area.

The Oregon Historical Society

For more than 125 years, the Oregon Historical Society has served as the state's collective memory, preserving a vast collection of photographs, books, maps, manuscript materials, films, oral histories, objects, and belongings. Their research library, museum, digital platforms, educational programming, and historical journal make Oregon's history available to all. They preserve the state's history and make it accessible to everyone in ways that advance knowledge and inspire curiosity about all the people, places, and events that have shaped Oregon.

Travel notes: Attendees will walk to and from The Oregon Historical Society (approximately 5-minutes from the Hilton Portland Downtown) and on the move for the self-guided tour. Washington Park Hike and International Rose Test Garden

The moderate hike will leave from the Washington Park Max light rail station and cover 2.5 miles over approximately 1 hour and 30 minutes. The hike will include a stop at the International Rose Test Garden, where attendees can take in the beautiful surroundings and concludes at the Hoyt Arboretum, where attendees can explore a diverse collection of trees from around the world. A local outdoors enthusiast will guide the hike.

Attire: Dress comfortably (rain gear if needed), wear appropriate footwear, and bring water.
Travel notes: Attendees will take the MAX Red Line, led by staff, from the Pioneer Square North stop (approximately 3-minute walk from the Hilton Portland Downtown) to the Washington Park Max Station. For the hike, expect a mix of paved and non-paved surfaces and some steep trails. Participants should have prior experience with hiking on uneven terrain. 



Scholarship contribution

To keep Putting Care at the Center as inclusive as possible, we have established a scholarship fund to cover registration, travel, and lodging for individuals who cannot afford them. Most scholarship support goes to individuals with lived experience of complex health and social needs.  

The average cost to support one person through the scholarship fund is $1,000. If you wish to do so, you may add support for the scholarship fund, in $25 increments, to the cost of your registration. Select your desired number of increments by clicking the “+” sign, and the total amount will be added to your registration payment.
