This presentation highlights a 35-year collaboration between a homeless service provider and a "federally qualified" health center (FQHC). Our changing approach over the years was designed to measurably improve health among individuals in PSH with recent results published in the American Journal of Public Health. We will discuss what has worked and considerations when replicating.

Cathy Crouch, LCSW, is Executive Vice-President of SEARCH Homeless Services, leading the agency in the implementation of evidence-based approaches since 1994. She is a trainer in the Motivational Interviewing Network of Trainers. Cathy is co-author of the best-selling Group Treatment for Substance Abuse: A Stages of Change Therapy Manual.

Phoebe Wong, LCSW is the Director of Housing at SEARCH Homeless Services. Phoebe has worked in frontline and leadership roles in homeless services for over 14 years. Her experience includes overseeing integrated care programs, which bring together social service and healthcare providers to serve residents in permanent supportive housing.
