Session Overview

Addressing homelessness requires an approach that addresses the needs of people experiencing homelessness, the causes of homelessness, and behavioral health supports. We will focus on long-term sustainability of Housing First Assertive Community Treatment and Intensive Case Management for unhoused individuals with severe mental illness and/or addiction in permanent supportive housing.

Jennifer Erasime, LCSW-S has more than ten years of clinical experience in both community and inpatient mental health settings, with a concentration on program development and service expansion. Her work has been committed to integrating recovery-oriented and person-centered care, in high acute clinical settings for individuals with serious and persistent mental illness, co-occurring substance use disorders, and complex medical conditions.

Presenter

Darilynn Cardona-Beiler, MSSA, LCSW, works with health systems to adopt clinical best practices and system integration to improve holistic health care treatment including what is often siloed as physical, mental, and substance use systems and treatment. Throughout her 25-year career in health care, social services, and community mental health, Darilynn has served the indigent, people experiencing homelessness, and underserved populations.
