Session Overview

HOST utilizes a comprehensive, multidisciplinary team that provides outreach-based treatment services reaching those least likely to engage in conventional physical and behavioral health service. We will discuss the statewide expansion of HOST; the challenges, successes, and differences across counties; and the successful collaboration with the local homeless crisis response system.

Tom Breitenbauch is the Housing Program Manager for Carelon Behavioral Health. He has served on several boards related to behavioral health and homeless services and has thirteen years’ experience in direct service implementation and contract management of permanent supportive housing, rapid rehousing, transitional housing, homeless outreach, and emergency shelter programs.

Presenter

Andrea Chiavarini (She/Her) works for Lifeline Connections, a nonprofit Behavioral Health agency in Vancouver, Washington. She provides withdrawal management services and medical treatment to people who are unhoused. Earlier in her career, she practiced general Obstetrics and Gynecology then transitioned into the field of Addiction Medicine in 2017.

Meta Hogan is the Program Manager for Homeless Outreach Stabilization and Transition at the Washington Healthcare Authority. Spanning a career of over twenty years working with unhoused individuals, she has served in a variety of roles including outreach worker, case manager, shelter director, peer supervisor, and program director.
