Housing First was initially developed to support persons with psychiatric disabilities live a meaningful life in an apartment setting rather than on the streets or in institutions. As the target population for housing first has evolved over time (e.g., older adults, transition age youth, etc.), so has the need for different support services to facilitate complex care management. This workshop will discuss how some evidence-based practices such as assertive community treatment have been adapted over time to meet the changing needs of tenants and how other interventions, such as fall prevention efforts for older adults, have been recently implemented. Audience members will be invited to share their experiences on how health and social services have been combined to support complex care management in their local contexts.
